Before you begin treatment with Monash IVF, there are a number of important steps that need your involvement to ensure a seamless entry into the program.
Firstly ask your general practitioner or your gynaecologist for a referral for both you and your partner to an IVF Specialist at Monash IVF. If you intend to use a referral from your general practitioner, ensure it is specified as ‘indefinite’.
There are other requirements that need to be satisfied as stipulated by the Assisted Reproductive Treatment Act 2008 that was introduced in January 2010 in particular Police Checks and Child Protection Order Checks.
Police Checks
You and your partner (if any) are required to provide a National Police Record Check. This applies to both interstate and overseas patients accessing treatment in Victoria. An Australian police statement will take a minimum of 10 working days to generate. An overseas police check may take quite a lot longer to come through especially from the UK and USA and possibly some other countries too.

When completing the application, it is important that in Section E you select ‘Option 1 Contact with children’ and write ‘ IVF Applicant’ in the free text box. If you do not do this, your police certificate cannot be accepted and you will be required to apply for a new Police Check.
Overseas patients
If you have never lived in Australia you will require a police statement from your home country, but not from Australia.
Overseas police statements that require a translation to English will be translated by Monash IVF’s service provider and please be informed that you will incur the translation cost.
Please note: Serious offences may result in a presumption against treatment and your treatment may be delayed until the matter is resolved. If you are concerned about specific offences and wish to discuss this, feel free to contact us for additional information.
Child Protection Order Checks
The ART Act 2008 requires that you give Monash IVF consent to apply to the Department of Human Services on your behalf for a Child Protection Order Check. The Child Protection Order Check statement will be sent directly to Monash IVF. All overseas and interstate patients must also apply for a Child Protection Order Check.

These checks need to be sighted by a Monash IVF Counsellor prior to commencing treatment or continuing treatment. If there are concerns a Counsellor will contact you.
Once you have received your preliminary investigation requests from your IVF Specialist you may contact Hawthorn on 03 94299188 or Clayton on 03 95432833 to organise a suitable time for a block booking to have all your preliminary clinical tests and counselling appointment undertaken.
Note: All couples accessing assisted reproductive treatment in Victoria must attend a mandatory counselling session prior to treatment in accordance with the ART Act 2008.
